NEW CADETS AND PARENTS

Information and Instructions

This web page is for use by all new cadets and parents.  It contains all the important and necessary forms, instructions and information needed for cadets entering the NJROTC program at Curtis High School. Cadets must already be enrolled in the program to submit these forms. (Check with your counselor to find out whether or not you are enrolled first.

Please complete the Online New Cadet Submittal form first, then follow the links to access each of the following forms, print the forms, complete them and submit them to the Naval Science Instructors as soon as possible.

·       Online New Cadet Submittal Form

• Cadet Data Sheet  (Contains required information to input cadet data into the Junior Uniform Maintenance computer System (JUMS)
• Drill Rifle Procedures (Must be signed in order for cadets to be allowed to handle drill rifles
• Health Risk Screening Questionnaire (must be renewed annually for cadet to be eligible for participation in strenuous activities and Physical Fitness awards)
• Standard Release Form  (Required for all major field trips)
• Harassment Statement   (Us Navy and Marine Corps policy regarding “zero tolerance” for harassment of any kind)
• Behavior Contract   (a statement of agreement regarding behavioral expectations for all cadets)
• Cadet Physical Examination Documentation (must be renewed annually for cadet to be eligible for participation in strenuous activities and  for Physical Fitness awards)
• Uniform Issue Form   (Print this form and use only for quarterly supply  inventories when directed by NSI’s)
